When there is change in personal information such as ID card number, contact address, mobile phone number and other information, Customers need to contact MBS to modify the information.
For changed information related to name, ID card number, date of issuance, place of issuance and contact address, Customers need to make procedures on changing information with MBS and the Vietnam Securities Depository (VSD).
Some instructions on preparing information modification applications:
1. Change ID card number:
- Application for changing information in accordance with MBS’s forms with sufficient reasons for adjustment, confirmation signature of the Customer, teller and supervisor of MBS.
- Original new ID card and one of the following documents:
- Confirmation from a competent agency (public security/military, etc.) on changing the old and new ID card of the Customer;
- Household book (written with old and new ID card numbers);
- Old ID card (notarized)/passport with old ID card number.
2. Change date of issuance for ID card:
- Application for adjusting information in accordance with MBS’s form with adequate reasons for adjustment, confirmation signatures of the Customer, teller and supervisor of MBS.
- Original new ID card of the Customer.
3. Change first name and last name of the Customer:
- Application for changing information in accordance with MBS’s forms with sufficient reasons for adjustment, confirmation signature of the Customer, teller and supervisor of MBS.
- Original new ID card of the Customer.
- Confirmation letter from the local authority or public security to verify the Customer’s first name and last name.
4. Change contact address or permanent residence address of the Customer:
- Application for changing information in accordance with MBS’s forms with sufficient reasons for adjustment, confirmation signature of the Customer, teller and supervisor of MBS.
- Original ID card of the Customer.
5. Change in first name, last name and information on ID card of the Customer:
- Application for changing information (Form No. 05/LK) with adequate information and reasons for adjustment of the Customer.
- New and old ID cards of the Customer.
- Confirmation from the local authority or public security on confirming the Customer’s first name and last name.
- A certified copy of account opening contract of the Customer (01 copy).
